Appointment Information  

Hours may change based on demand and / or circumstances beyond our control. Please call (757) 594-7661 to confirm current hours and to schedule an appointment.  Appointments cannot be made by email.

First appointment in Health & Wellness Services?

New Patients:  Please arrive about 10-15 minutes early to fill out the necessary paperwork or save time and fill out the Patient Consent to Treat and Financial Responsibility Agreement before you arrive.

• If you have been here before, and
• Have changed your local address / telephone number, or
• Have had any changes to your medical health/status,
• please let us know by filling out and bringing in a Patient Information Update Form

Students are seen by appointments and same day appointments are usually available.

Please be respectful of others and be on time for your appointment. In order to insure that all patients are seen at their scheduled time, being 15 or more minutes late for your appointment will necessitate  rescheduling.

Appointment Fees:

Nurse Practitioner (FNP) and Physicians   Fees: Payment is accepted by cash, check and Captains flex card.

Lab work is an additional cost depending on what is required.  Labs can be billed to insurance.
