Current Students

When a student, 18 years or older, visits the clinic, all information relating to that visit is documented and is considered protected medical information.   Without a student's written consent, no information may be discussed or released to anyone.  If a student wants information or records released, the student may give written consent by completing one of the two forms below.

Graduates and Alumni

The Student Health Center maintains patient records for six years after a student graduates from Christopher Newport University. After six years health records are destroyed. If you need information from your health record that is less than six years old you may request it by completing the Request and Consent to Release Medical Information form.
